Chronic Disease Management

Adapt • Improve • Succeed

Our program is designed to educate and support individuals who are at high-risk for or are currently managing medical conditions. It addresses the individual as a whole, and any health issues he or she might have due to diabetes, cardiopulmonary disease, neurological conditions, chronic pain, and other chronic conditions.

Our focus is on lifestyle and behavior change. The program includes an exercise component paired with health coaching to ensure well-rounded guidance. Our long-term goal is for participants to be able to perform activities of daily living (ADLs) and obtain longevity while managing their conditions.

Program Includes:

  • 12 week membership
  • Orientation
  • Pre-Program Health Assessment
  • Exercise Plan
  • Weekly Exercise Sessions with a Certified Personal Trainer with medical specialties
  • Bi-Weekly Meetings with a Health Coach
  • Post-Program Health Assessment
  • Exit Plan Appointment

A physician’s referral must be obtained before enrollment into the program.
