Genetic Risk Assessment
Genetic Risk Assessment
Knowing your risk for breast cancer is the first step in empowering yourself against the disease. When you have a mammogram at the Breast Center, you can also receive a genetic risk assessment at no cost.
This assessment evaluates your personal and family risk factors for developing breast cancer. Women who are at a higher-than-average risk of developing the disease may be referred for a genetic consultation to determine if genetic testing is needed to help them make informed health care decisions. Our goal is to prevent breast cancer from occurring.
To participate in the genetic risk assessment, you will be asked to provide information on your personal and family history of cancer.
Please print and complete this form before your mammography appointment.
